Between 1592 and 1597, the first sacred building was erected in Surviliškis, and in 1938, the nearby village of Bučioniai was assigned to the area. From 1637 to 1640, the Bishop of Žemaitija, J. Tiškevičius, financed the construction of a new church from his own funds and donated 14 valaks of land to it. Around 1777, a parish school operated next to the church. In 1791, a wooden Church of the Crucified Jesus was built here, featuring elements of Baroque and Classicism architecture. In 1865, priest L. Urbonavičius, who served in the church, was accused of supporting rebels and was exiled to Tomsk Province that same year. From 1908 to 1914, a branch of the Lithuanian Catholic Temperance Society operated in the Surviliškis church. Visitors can enter the church only during services.
